With life expectancies 10 years below other Louisville neighborhoods, Smoketown Family Wellness Center strives to establish healthy lifestyles from the beginning of life, providing education and programming for children and their families in a safe community environment.
The Smoketown Family Wellness Center (SFWC) builds a culture of health by providing clinical care to children in a pediatric clinic and community-based environment with a focus on healthy lifestyle behaviors for the entire family. SFWC will improve health outcomes by addressing the many factors that impact health in one setting. SFWC works with parents to support them with the knowledge and resources they need.
The SFWC is an integrated healthcare model to support parents to raise children to be healthy in mind, body, and spirit. Opening in early 2018, the Center will provide medical care for children and healthy lifestyle support for the whole family in a community-based setting.

The Center is in the heart of Smoketown. We are located at the ground level of the Old Presbyterian Community Center (OPCC) — a building that has a lot of love baked into its walls. The OPCC was established in 1929 and served as a beloved space to many Smoketown residents in Sheppard Square providing enrichment activities for youth and families.
